NEW INTEGRATED HEALTH AND SOCIAL CARE TEAM FOR WALKDEN
Older people in Walkden and Boothstown are benefiting from a new approach to providing services introduced by Salford Primary Care NHS Trust and Salford City Council.
Traditionally NHS and Local Authority services are provided separately with little contact or co-ordination and much duplication. Salford's response to this unacceptable situation has been to set up the Walkden Integrated Health and Social Care Team (Integrated Team) based at Walkden Clinic (Bridgewater Road).
The Integrated Team has a very different approach to helping patients, with community nursing staff and social care staff working together from the same location and often making joint home visits where appropriate. The result: less duplication and more effective care and treatment, helping older people remain independent longer.
The Integrated Team provides health and social care services for older people in Walkden and parts of Boothstown. It is the first such team in Greater Manchester and the first of several teams planned for Salford. The Integrated Team consists of 26 staff: 17 District Nurses, four Social Workers, two Admin Officers, an Occupational Therapist and a Community Matron.
In addition there is a local, named NHS mental health worker.
